During your first visit we will take a set of digital x-rays to evaluate your dental health, our hygienist will assess and chart the health of your gum tissues and clean your teeth, then Dr. Schultz will perform a detailed dental evaluation, oral cancer examination, review your medical and dental histories and discuss all of the findings with you.  

We value your time and will make every effort to see you promptly. You can download the necessary new patient form below so you don’t have to spend extra time filling out forms when you arrive. If you have dental insurance, we will verify your benefits at your first visit.

We ask that new patients try to arrive about 20 minutes before their appointment time so that we can process the new patient information form and other related documents without diminishing the time spent with Dr. Schultz.
